
CHIRS provides a variety of long-term, supportive
housing options. The Residential Team works
collaboratively with the individual receiving services,
their family, clinical consultants and Adult Day
Services to deliver comprehensive, individualized
programs and support.
Residential Services are designed to provide flexible,
individualized supports, which maximize functional
independence in all areas of community living. The
type and amount of support varies with the unique
abilities and needs of each individual. Service Plans
are developed and implemented in collaboration with
each individual and their family.
CHIRS offers a range of living options, within a
variety of residential settings. We make every effort
to ensure that each individual resides in an
environment best suited to his or her current needs.
Aldebrain Towers
Located in Scarborough, within an integrated
community apartment complex, CHIRS provides
twenty-four-hour support services as well as variable
support with access to staff on a twenty-four-hour
basis. In keeping with our commitment to provide
flexible, comprehensive support, individuals may live
alone, or with one or more roommates. Every effort
is made to ensure compatibility of roomates. The
four-bedroom apartments are fully wheelchair
accessible, bright and very spacious. Each individual
has a private bedroom and shares a washroom with
one other person. Each person is responsible for
furnishing his or her own bedroom while CHIRS
provides furnishings for the common areas.
Residential Facilitators provide support to all
residential clients.
Individuals living in the one and two-bedroom
apartments have access to CHIRS staff on a twenty-
four-hour basis. Support services are negotiated with
each individual in order to facilitate participation in
activities within their home and the community.
These units are not typically wheelchair accessible.
The "Drop-In-Center" is centrally located and is
available for all CHIRS residents to use. This area is
used for casual and planned gatherings, group
meals, meetings or just a place to relax. Individuals
plan and participate in parties, potluck dinners,
movie nights, card games and a variety of other
activities.
St. George's Residence
Twenty-four-hour supervision is provided to seven
men at this CHIRS-owned residence in Etobicoke.
The spacious bungalow is fully wheelchair
accessible and close to public transit and shopping.
The large yard provides opportunities for gardening,
barbequing and enjoying the outdoors. Each of the
residents has a private room that they furnish
themselves.
Finch Residence
Variable support is provided to individuals residing at
the Finch Residence. Five self-contained apartments, three of these fully accessible, facilitate an
increased level of independence. The residence is
located on the fourth floor of CHIRS' head office,
centrally located with easy access to public transit
and many local amenities.

Residential Facilitators provide the direct support to
residents in collaboration with the individual, their
family and the clinical team. Each resident has a
Primary Residential Facilitator who is responsible for
monitoring the services provided.
Staff work in teams to provide support to a small
group of residents. A Service Coordinator who
reports directly to the Residential Manager or
Program Director supervises each team, including
Senior Facilitators. Clinical expertise is accessed
through CHIRS' Neuropsychologist, Social Worker
and consulting Neuropsychiatrist. CHIRS routinely
collaborates with community partners in order to
access services such as physiotherapy and
occupational therapy as required.
Activities
Each individual is involved in home and community
activities, in keeping with their abilities and interests.
CHIRS facilitates the development of positive
everyday routines, which promotes the attainment of
personal goals. Individuals may participate in a
range of recreation and leisure activities,
employment, education, exercise and social
commitments. Many residents choose to access the
diverse and exciting programs offered through
CHIRS' Adult Day Services.
Family and Friends
The extent of involvement of family and friends
depends on the wishes of the resident and his or her
family. CHIRS is committed to facilitating and
encouraging these relationships. There are numerous
opportunities for involvement including casual
visits at the residence, visits to the homes of friends
and families, special events and service planning
meetings.
