Practical Assessments for Real Life

For the past 5 years, CHIRS’ Neuropsychologist and Clinical Director, Dr. Carolyn Lemsky, has been offering independent medical examinations and assessments for rehabilitation on a limited basis. Now, with the addition of a Neuropsychological Fellow, CHIRS has expanded capacity for neuropsychological assessments.

Dr. Lemsky has over 15 years of experience in providing rehabilitation and assessment services in medical-legal, inpatient and community-based settings. She is a Diplomate of the American Board of Professional Psychology, American Academy of Clinical Neuropsychology.

Fees

Assessments are billed at an hourly rate consistent with Ontario Psychological Association’s recommendations.

Test batteries and reports are tailored to the purpose of the examination. Written estimates of costs are provided before the examination. Signed approval for billing is required before the date of testing.
